I’m leaving hospital and I don’t think I’ll be able to cope at home
If you are worried about coping at home when you leave hospital, it may be possible to adapt your home so you live more comfortably and independently.
The hospital can refer you to the social services team at North Yorkshire County Council for advice and support. If you want to speak to the North Yorkshire County Council, you can call them on 01609 780780.
- An occupational therapist will then arrange to visit you to see what sort of help you need
- They will decide if your home can be adapted to make things easier for you
- If your home is unsuitable, we will work with housing providers in Ryedale to help you find a solution
I’m about to leave hospital and have nowhere to go
If you have nowhere to go, please tell the hospital nursing staff as soon as possible.
- The nursing staff can then let the hospital discharge team know you’ll be homeless, and they will be able to help you
- The discharge team may refer you to our housing advice team
- We will then assess your situation and may be able to offer you temporary accommodation
- If you are going to leave hospital in the next 56 days, and have no home to return to, our housing options team will support you whilst we decide if we are able to re-house you
- We’ll also offer you advice and support
